Virtual Consultation Request

This consultation will include a review of the various pre-treatment questionnaires completed by the patient (or their parents) on our website and a one hour clinical interview conducted by Dr. Monastra. The purpose of the interview is to review the kinds of problems being experienced by the patient, the onset of symptoms, the kinds of functional problems caused by these symptoms, and response to any prior treatments. The session will also include a review of the patient's lifestyle habits (eating, sleeping, exercise, use of alcohol, nicotine and other psychoactive substances) and their impact on attention and mood.

During this consultation, Dr. Monastra will review the results of medical, neuropsychological, and QEEG assessments conducted since the New Patient Assessment Consultation and develop a comprehensive treatment plan for the patient.

This consultation will include a review of post-treatment questionnaires completed by the patient (or their parents), as well as the results of medical and QEEG assessments conducted in order to evaluate clinical progress. The session will also include the revision of the previous treatment plan, as needed.
